Overview
Understanding knee pain
Knee pain is the most common reason patients see an orthopedic surgeon. In most adults over 45 it is osteoarthritis; in younger, active people it is more often a meniscus or ligament injury. The right treatment depends entirely on which it is, and the majority of knee pain is managed without an operation.

When to get knee pain checked
- Knee pain that wakes you at night or continues at rest
- The knee locks, catches or will not fully straighten
- The knee gives way when turning or on uneven ground
- Swelling that keeps coming back
- A bow-leg or knock-knee that is visibly getting worse
Emergency: A hot, red, swollen knee with fever, or a knee you cannot bear weight on after an injury, needs the nearest hospital the same day.
Common Causes
What usually causes knee pain
The cause decides the treatment, which is why the consultation starts with a proper examination rather than a prescription.
Osteoarthritis
Gradual wear of the joint cartilage. Pain on stairs and when getting up from a chair, morning stiffness, difficulty sitting on the floor, and often a slowly increasing bow-leg.
Meniscus tear
From a twisting injury in younger people, or degeneration in older knees. Pain along the joint line, catching or locking, and swelling the day after activity.
Ligament injury (ACL, PCL)
A twist with a pop and rapid swelling, followed by a knee that gives way on turning or on uneven ground. Often walked off at the time and only noticed later.
Kneecap (patellofemoral) pain
Pain at the front of the knee on stairs, squatting or after sitting for long. Common in younger women and in people who have recently increased training.
Post-traumatic and inflammatory arthritis
An old fracture into the joint, or rheumatoid arthritis, can wear the knee out earlier than normal and changes how it should be treated.
Treatment
How knee pain is treated
Step 1
Treatment without surgery
Tried first, and enough for most patients.
- Quadriceps and hip strengthening exercises, the most effective non-surgical treatment for knee arthritis
- Weight reduction where relevant: every kilo lost takes several kilos of load off the knee
- Structured physiotherapy for meniscus, ligament and kneecap problems
- Short courses of medicine for flare-ups, with long-term painkiller use reviewed
- Injections in selected cases, with their limits explained honestly
Step 2
When surgery is genuinely needed
Advised only when non-surgical treatment has been properly tried, or the problem will not heal without it.

FAQ
Knee Pain: common questions
Does knee pain always mean I will need a knee replacement?+
No. Most knee pain, including most early and moderate arthritis, is managed with strengthening exercises, weight management, physiotherapy and sensible use of medicines. Replacement is advised only when the X-ray shows advanced arthritis and the pain limits daily life despite proper non-surgical treatment.
What tests are needed for knee pain?+
A standing (weight-bearing) X-ray of both knees is usually the first and most useful test for arthritis. An MRI is mainly needed when a meniscus or ligament injury is suspected, typically in younger patients after a twisting injury. It is rarely needed to diagnose arthritis.
Are knee injections worth trying?+
A steroid injection can settle an arthritis flare for weeks to a few months, but frequent repeated injections are not recommended. Other injections have mixed evidence. Whether an injection makes sense for your knee, and what it can and cannot do, is explained before anything is given.
Should I exercise if my knee already hurts?+
Usually yes, with the right exercises. Strong thigh and hip muscles take load off the knee joint, and a structured programme is one of the best-proven treatments for knee arthritis. Exercises that cause sharp pain or swelling should be modified, not pushed through.
When is knee replacement the right choice?+
When pain limits walking, sleep and daily activities, when non-surgical treatment has been properly tried, and when the X-ray confirms advanced joint damage. Age alone is not the deciding factor. If replacement is advised, you are told which implant, why, the expected recovery and the realistic outcome.
