Overview
Understanding hip pain
True hip joint pain is usually felt in the groin or the front of the thigh, not the buttock. Pain at the back of the hip often comes from the lower back instead. Getting that distinction right is the first job of the consultation, because hip arthritis, avascular necrosis (AVN) and a missed hip fracture each need a different and often time-sensitive plan.
हिंदी: कूल्हे का दर्द
When to get hip pain checked
- Groin or front-of-thigh pain when walking or standing up
- Difficulty putting on socks or shoes, or cutting toenails
- A limp, or one leg looking shorter than the other
- New hip pain after a course of steroid medicines
- Hip pain that disturbs sleep
Emergency: An elderly person who falls and then cannot stand or bear weight may have a hip fracture and needs the nearest hospital the same day.
Common Causes
What usually causes hip pain
The cause decides the treatment, which is why the consultation starts with a proper examination rather than a prescription.
Hip osteoarthritis
Groin pain on walking, stiffness putting on socks and shoes, and difficulty sitting cross-legged. It progresses gradually, often over years.
Avascular necrosis (AVN)
Loss of blood supply to the ball of the hip, most often in adults aged 25 to 50. Linked to steroid medicines and heavy alcohol use. Early diagnosis keeps more options open.
Fracture that has not united
A hip fracture in an older patient treated with bed rest only, or fixation that has failed. These can usually still be treated, often with a hip replacement.
Inflammatory arthritis
Ankylosing spondylitis and rheumatoid arthritis can damage the hip at a younger age and are managed together with a rheumatologist.
Pain referred from the spine, or bursitis
Buttock pain spreading down the leg is often from the back; pain on the outer side of the hip when lying on it is often bursitis. Neither is hip arthritis.
Treatment
How hip pain is treated
Step 1
Treatment without surgery
Tried first, and enough for most patients.
- Activity modification and a walking stick held in the opposite hand
- Physiotherapy to keep hip movement and strengthen the muscles around it
- Weight management to reduce load through the joint
- Medicines for pain, with long-term use reviewed for safety
- In early AVN, close monitoring with MRI and joint-preserving options where suitable
Step 2
When surgery is genuinely needed
Advised only when non-surgical treatment has been properly tried, or the problem will not heal without it.

FAQ
Hip Pain: common questions
How do I know whether my pain is coming from the hip or the back?+
Hip joint pain is typically in the groin or front of the thigh and worse on walking and turning the leg. Back pain more often sits in the buttock and lower back and may run down the back of the leg. A clinical examination, rotating the hip and checking the spine, together with an X-ray of the pelvis, usually settles it.
What is AVN of the hip and how urgent is it?+
Avascular necrosis is death of bone in the ball of the hip due to loss of blood supply. It is time-sensitive: before the ball collapses, joint-preserving options may still be possible; after collapse, hip replacement is usually needed. If you have been diagnosed with AVN, have it assessed promptly rather than waiting for the pain to worsen.
Is hip replacement a major operation? How long is recovery?+
It is a planned operation with a hospital stay of a few days. Most patients walk with support within a day or two, use a walker or stick for some weeks, and return to normal daily activity over a few months. The exact plan depends on your age, health and the reason for surgery.
Can an old hip fracture still be treated?+
Often yes. A hip fracture that has not united, or has collapsed, can usually be treated surgically, frequently with a hip replacement. The longer it is left, the weaker the muscles and bone become, so earlier assessment gives a better recovery.
Which test is needed for hip pain?+
An X-ray of the pelvis showing both hips is the first test. An MRI is useful when AVN or a hidden fracture is suspected and the X-ray looks normal. Bring any earlier imaging too, because the comparison shows how the hip is changing.
