Overview

Understanding shoulder pain

Shoulder pain is one of the most frequently misdiagnosed orthopedic problems. Much of what is labelled frozen shoulder turns out to be a rotator cuff tear, and the two are treated very differently: a frozen shoulder usually recovers, while a cuff tear does not heal on its own and can enlarge. A careful examination, sometimes with an ultrasound or MRI, separates them.

When to get shoulder pain checked

  • Night pain when lying on the affected shoulder
  • Weakness lifting or reaching overhead
  • A shoulder that has dislocated more than once
  • Stiffness that keeps getting worse over months
  • Unable to lift the arm after a fall

Emergency: A shoulder that has just dislocated, or a deformed shoulder after a fall, needs the nearest hospital straight away for X-ray and reduction.

Common Causes

What usually causes shoulder pain

The cause decides the treatment, which is why the consultation starts with a proper examination rather than a prescription.

1

Rotator cuff tear or tendinopathy

Pain on lifting the arm, night pain when lying on that side, and weakness reaching overhead. Can follow a fall or develop from years of overhead work.

2

Frozen shoulder

Gradually increasing pain and stiffness in all directions. Much more common in people with diabetes or thyroid problems. Usually recovers, but slowly.

3

Recurrent dislocation and instability

A shoulder that has come out once is likely to keep slipping out, especially in younger people. Each episode can damage the joint further.

4

Shoulder arthritis

Grinding, deep aching pain and loss of movement in older patients, or after an old fracture. Advanced cases may need shoulder replacement.

5

Impingement and bursitis

Pain in an arc as the arm is raised, often from overuse. Usually settles with physiotherapy and activity change.

Treatment

How shoulder pain is treated

Step 1

Treatment without surgery

Tried first, and enough for most patients.

  • Targeted physiotherapy: different exercises for a frozen shoulder and a cuff problem
  • An injection in selected cases of frozen shoulder or bursitis
  • Activity and work modification to reduce overhead strain
  • Diabetes control, which directly affects frozen shoulder recovery
  • Short courses of pain medicine to allow exercise

Step 2

When surgery is genuinely needed

Advised only when non-surgical treatment has been properly tried, or the problem will not heal without it.

Where To Consult

Get shoulder pain assessed in Jaipur or at a district OPD

Consultation and follow-up are available in Jaipur and at three district OPDs. Surgery, if needed, is always performed in Jaipur.

FAQ

Shoulder Pain: common questions

Is it a frozen shoulder or a rotator cuff tear?+

In a frozen shoulder, movement is restricted in every direction, even when someone else tries to move the arm. In a cuff tear, the arm can usually be moved passively but you struggle to lift it yourself and it feels weak. An examination distinguishes them, and an ultrasound or MRI confirms a tear.

Will a rotator cuff tear heal on its own?+

A torn tendon does not reattach by itself, and tears can enlarge over time. Some small or degenerative tears in older patients do well with physiotherapy. Larger tears, and tears in active patients, are often better repaired before the muscle weakens, usually by keyhole (arthroscopic) surgery.

How long does a frozen shoulder take to get better?+

It typically improves over many months, sometimes longer, especially with diabetes. The right physiotherapy and sometimes an injection shorten the painful phase. Surgery is needed only for the minority that do not improve with proper treatment.

My shoulder has dislocated more than once. Do I need surgery?+

Often yes, particularly in younger and active patients. Repeated dislocation damages the cartilage rim and bone of the socket, and the risk of further episodes stays high. Stabilisation surgery, usually arthroscopic, is discussed after an examination and MRI.

Is shoulder replacement an option if I have been told nothing more can be done?+

Sometimes. Shoulder replacement, including reverse shoulder replacement, is offered at relatively few centres, so patients are often told nothing more can be done for severe arthritis or an irreparable cuff tear. X-rays and a CT scan show whether replacement is an option for you.
