Overview

Understanding back pain

Most back pain is mechanical, from strained muscles and ligaments or age-related wear in the spine, and it settles with time, activity and the right exercises. The orthopedic consultation is about finding the minority of cases that need more: an osteoporotic fracture, a trapped nerve, or pain that is actually coming from the hip. Dr. Agrawal is a joint replacement and arthroscopy surgeon, not a spine surgeon: if spine surgery is needed, you will be told so and referred to a spine specialist.

When to get back pain checked

  • Leg pain below the knee lasting more than six weeks
  • Weakness in the leg or foot, or difficulty lifting the toes
  • Back pain after a fall in an older person
  • Back pain with fever, unexplained weight loss or a history of cancer
  • Night pain that does not ease with rest or change of position

Emergency: Loss of bladder or bowel control, or numbness around the groin and buttocks, is an emergency: go to the nearest hospital immediately.

Common Causes

What usually causes back pain

The cause decides the treatment, which is why the consultation starts with a proper examination rather than a prescription.

1

Mechanical back strain

Pain in the lower back after lifting, bending or long hours sitting or driving. The most common cause, and it usually improves within weeks.

2

Disc problems and sciatica

Pain running from the back down the leg, often below the knee, sometimes with tingling or numbness. Most settles without surgery.

3

Osteoporotic compression fracture

Sudden back pain in an older person after a minor fall, lifting or even a cough. Often missed, and a warning sign that the bones need treatment.

4

Hip arthritis mistaken for back pain

Pain around the buttock and thigh that is actually from the hip joint. Treating the back will not help; the hip needs assessing.

5

Posture and occupational strain

Repeated bending, heavy lifting, tractor driving and prolonged standing load the spine. Changing how the work is done often matters more than medicine.

Treatment

How back pain is treated

Step 1

Treatment without surgery

Tried first, and enough for most patients.

  • Staying active: bed rest slows recovery from most back pain
  • Core-strengthening and flexibility exercises with a physiotherapist
  • Short courses of pain medicine to allow movement
  • Lifting technique, posture and work adjustments
  • Bone density assessment and osteoporosis treatment where fractures are suspected

Step 2

When surgery is genuinely needed

Advised only when non-surgical treatment has been properly tried, or the problem will not heal without it.

Spine surgery for disc prolapse, canal stenosis or instability is not performed here. Where it is needed, you will be referred to a spine surgeon.

Where To Consult

Get back pain assessed in Jaipur or at a district OPD

FAQ

Back Pain: common questions

Do I need an MRI for back pain?+

Usually not at first. Most back pain improves within a few weeks, and MRI often shows age-related changes that are not causing the pain. An MRI is useful when there is leg weakness, pain running below the knee that is not settling, or warning signs such as fever or weight loss.

Does Dr. Agrawal perform spine surgery?+

No. Dr. Agrawal is a joint replacement, arthroscopy and trauma surgeon. He assesses back pain to find the cause, manages mechanical pain and osteoporotic fractures, and identifies when the hip rather than the spine is the problem. If you need spine surgery, you will be told plainly and referred to a spine specialist.

Should I rest in bed with back pain?+

No. For most back pain, staying gently active speeds recovery, while prolonged bed rest weakens the muscles and slows it down. Short periods of rest during a bad flare are fine, followed by a gradual return to walking and normal activity.

Could my back pain actually be coming from my hip?+

Yes, and it is commonly missed. Hip arthritis can cause pain in the buttock and thigh that feels like back pain. If rotating the hip reproduces your pain and walking is limited, an X-ray of the pelvis can confirm it, and treating the hip resolves the pain.

When is back pain an emergency?+

Loss of control of the bladder or bowels, numbness around the groin, or rapidly increasing weakness in the legs need emergency care at the nearest hospital immediately. Back pain after a significant fall or with high fever should also be seen the same day.

Where can I consult Dr. Hemendra Agrawal for back pain?+
