Overview

Understanding arthritis

Arthritis means damage to the smooth cartilage that lines a joint. The most common form, osteoarthritis, affects the knees, hips and shoulders and progresses gradually. Inflammatory types such as rheumatoid arthritis behave differently and are managed together with a rheumatologist. Most arthritis is managed without surgery for years; joint replacement is for when a joint has stopped working despite proper treatment.

When to get arthritis checked

  • Pain that limits how far you can walk
  • Pain at night or at rest
  • A joint that is becoming visibly bent or deformed
  • Painkillers that no longer control the pain, or are needed daily
  • Several joints swollen at once, with long morning stiffness

Emergency: A single hot, red, swollen joint with fever can be an infection and needs the nearest hospital the same day.

Common Causes

What usually causes arthritis

The cause decides the treatment, which is why the consultation starts with a proper examination rather than a prescription.

1

Osteoarthritis

Age-related and load-related wear, most often in the knees and hips. Pain with activity, stiffness after rest, and gradual loss of movement and alignment.

2

Rheumatoid and inflammatory arthritis

The immune system attacks the joint lining, often in several joints at once, with morning stiffness lasting over an hour. Medical treatment by a rheumatologist comes first.

3

Post-traumatic arthritis

Arthritis that develops years after a fracture into a joint, or after an untreated ligament or meniscus injury in the knee.

4

Arthritis after AVN

When the ball of the hip or shoulder collapses after loss of blood supply, the joint wears out quickly and usually needs replacement.

5

Gout and crystal arthritis

Sudden, very painful attacks in one joint, often the big toe or knee. Diagnosed with blood tests and joint fluid, and treated medically.

Treatment

How arthritis is treated

Step 1

Treatment without surgery

Tried first, and enough for most patients.

  • Strengthening exercise and staying active, adjusted to the joint involved
  • Weight management, especially for knee and hip arthritis
  • Physiotherapy, walking aids and supportive footwear
  • Medicines for flares, with long-term painkiller use reviewed for stomach and kidney safety
  • Referral to a rheumatologist when the pattern suggests inflammatory arthritis

Step 2

When surgery is genuinely needed

Advised only when non-surgical treatment has been properly tried, or the problem will not heal without it.

FAQ

Arthritis: common questions

Can arthritis be cured?+

Worn cartilage does not grow back, so osteoarthritis cannot be reversed. But its symptoms can be controlled well for years with exercise, weight management and sensible use of medicines. When a joint is badly worn and limiting your life, joint replacement replaces the damaged surfaces and usually removes the arthritic pain.

Is there a right age for joint replacement?+

There is no fixed age. The decision depends on how much the joint limits you, what the X-ray shows, and your overall health. Younger patients are generally encouraged to delay surgery while non-surgical treatment works; older patients should not be told they are simply too old if they are otherwise fit.

How do I know if my arthritis is osteoarthritis or rheumatoid?+

Osteoarthritis usually affects one or two large weight-bearing joints and is worse with use. Rheumatoid arthritis tends to affect several joints on both sides, including the small joints of the hands, with prolonged morning stiffness. Blood tests help confirm it, and inflammatory arthritis is treated medically first.

Are long-term painkillers safe for arthritis?+

Occasional use is generally fine, but daily long-term use of anti-inflammatory painkillers can affect the stomach, kidneys and blood pressure, especially in older patients. Part of every arthritis consultation is reviewing what you are taking and whether a safer plan is possible.

Can both knees be replaced at the same time?+

In suitable patients, yes. Whether both knees are replaced together or in two stages depends on your general health, heart and lung fitness, and how severe each knee is. This is decided on assessment, with the risks of each approach explained.
